What is exported

India exports disposable syringes and needles, catheters, cannulae, surgical and dental instruments and electro-medical equipment. In 2025-26 India exported about US$ 1.5 billion under HS 9018; the USA led, followed by Germany, the Netherlands, Brazil and China. Every importing country treats these as medical devices and requires registration before sale.

Certificates

  • CDSCO manufacturing licence (Medical Devices Rules, 2017)RequiredNeeded to make medical devices in India.
  • ISO 13485RequiredMedical device quality system; required by almost every importer and regulator.
  • CE marking under EU MDR 2017/745Often askedRequired for the EU; needs a notified body for most classes above Class I.
  • US FDA registration, listing and 510(k)Often askedRequired for the US market; Class II devices usually need 510(k).
  • Free sale certificate (CDSCO)Often askedMany importing countries ask for it to register a device.
  • WHO PQS (for some immunisation devices)Often askedAuto-disable syringes supplied to UN programmes need WHO PQS prequalification.

Shipping, storage and shelf life

Container load

Disposables are light and bulky: a 40-ft container fills by volume. Equipment ships on pallets or in crates.

How it travels

Sea freight for volume; air for urgent and high-value items.

Storage

Clean, dry storage away from heat; keep sterile packs intact.

Shelf life

Sterile disposables typically carry a 3–5 year expiry set by the maker's validation.

What goes on the pack and carton

  • Device name, model, lot, expiry, sterilisation method, "single use" and maker's name and address.
  • EU: CE mark with notified-body number, EU representative, UDI and ISO 15223-1 symbols.
  • USA: FDA labelling rules (21 CFR 801) and UDI.
Where to get it tested

NABL-accredited labs test biocompatibility, sterility and device performance (search 'medical devices' in the NABL directory). CDSCO-notified medical device testing laboratories are listed on the CDSCO site. The Kalam Institute of Health Technology at AMTZ, Visakhapatnam, also supports testing and certification.

Benefits of exporting this product

Where to register

  • EEPC IndiaMedical, surgical and other instruments are listed under EEPC in Appendix 2T; take your RCMC from EEPC.eepcindia.org ↗
  • CDSCO / State Licensing AuthorityManufacturing licence under the Medical Devices Rules, 2017; CDSCO also issues the free sale and market standing certificates many importing countries ask for. Electronic medical equipment can also join ESC; disposable syringes and medical plastics are under PLEXCONCIL in Appendix 2T.cdsco.gov.in ↗

Quality and checks

  • EU: CE marking under the Medical Device Regulation (EU) 2017/745, with a notified body for most classes above Class I, an EU authorised representative and EUDAMED registration.
  • USA: FDA establishment registration and device listing; most Class II devices need 510(k) clearance.
  • ISO 13485 quality system, sterilisation validation (ISO 11135 or ISO 11137) and biocompatibility (ISO 10993) for sterile and patient-contact items.
How to start small

A trader can start with simple Class I or low-risk items (surgical instruments, dressings-related tools) for Gulf, African or South Asian distributors, buying from an ISO 13485 maker that holds a CDSCO licence and can give a free sale certificate. Plan for registration time in the buyer country.

Where new exporters go wrong

  • Shipping before the device is registered in the buyer country — it cannot be sold or cleared.
  • Sterile goods without validated sterilisation records and expiry dates on labels.
  • Buying from an unlicensed workshop — buyers and regulators check the CDSCO licence.

Step by step

How to export medical and surgical instruments

India is a growing exporter of medical devices, strongest in disposables (syringes, needles, catheters) and in surgical and dental instruments, and it is building device parks and a PLI scheme to move into higher-value equipment. The USA, Germany, Ireland, the Netherlands and China lead world trade; China and Pakistan compete on instruments and disposables. Demand comes from ageing populations, hospital expansion in the Gulf, Africa and South-East Asia, and buyers looking for alternatives to China. Openings are in registered disposables, orthopaedic and dental tools and affordable diagnostic equipment.

  1. 01Pick devices and markets

Start with low-risk devices and markets with simpler registration.

  2. 02Check the maker

CDSCO licence, ISO 13485 certificate and sterilisation records.

  3. 03Register in the buyer country

Through a local importer or authorised representative; supply technical files.

  4. 04Get export documents

Free sale certificate from CDSCO where asked.

  5. 05Produce and release

Batch records, sterility results and certificate of analysis.

  6. 06Ship and claim

File the shipping bill; claim RoDTEP and drawback.

  7. 07Payment

LC or advance for new distributors; tenders often pay after delivery, so use ECGC cover.

Questions

Questions about exporting medical and surgical instruments

Can a trader export medical devices?
Yes, as a merchant exporter buying from a licensed maker, but the device must be registered in the buyer country.
What is ISO 13485?
The international quality-management standard for medical device makers; almost every buyer asks for it.
Which council?
EEPC India lists medical, surgical and other instruments; ESC covers electronic medical equipment and PLEXCONCIL covers disposable syringes and plastic medical disposables.
How long does EU CE marking take?
For classes needing a notified body it often takes many months; plan early and check notified-body capacity.
What RoDTEP applies?
Most 9018 lines carry 0.5% of FOB; cannulae carry 0.9%.
